Weight gain in perimenopause and menopause is common, often at the waist. Menopause and HRT have not been shown to be the main cause of the extra pounds.

#### [Does HRT cause weight gain? What the trials show ](/articles/will-hormone-replacement-therapy-cause-weight-gain)

HRT does not cause weight gain, according to a Cochrane review of 28 trials. Early bloating and fluid retention can feel like weight gain, and both usually settle.

### Weight and diet: the overview

Many women gain weight in their late 40s and 50s and blame the menopause. The Menopause Society's answer is more specific: there is no scientific evidence that menopause or hormone therapy causes the extra kilos. Ageing and lifestyle drive most of it, including the slow loss of muscle that starts decades earlier, less activity, poorer sleep and stress ([The Menopause Society](https://menopause.org/for-women/sexual-health-menopause-online/changes-at-midlife/changes-in-weight-and-fat-distribution)).

What menopause does change is where fat goes. As oestrogen falls, the body stores more of it around the middle, and that waist fat carries more risk for heart disease and type 2 diabetes than fat on the hips and thighs.

### Food and bones

Oestrogen helps the body hold on to calcium, so bone loss speeds up after menopause. Calcium-rich foods matter (dairy, tinned fish with bones, green leafy vegetables, nuts and seeds), and UK health advice is that adults consider a daily vitamin D supplement, especially from October to March ([NHS advice on supplements](https://www.nhs.uk/conditions/vitamins-and-minerals/vitamin-d/)).

### What the guides here cover

Articles in this topic cover eating patterns for menopause, protein and strength training to keep muscle, what to make of menopause diet plans and supplements, and whether HRT affects weight. [Menopause weight gain and belly fat](/articles/menopause-weight-gain) explains why the waist changes and which food and movement steps have published support. The trial results on HRT and the scales are in [Does HRT cause weight gain?](/articles/will-hormone-replacement-therapy-cause-weight-gain)

Our editors' view: put strength training ahead of a new diet. Keeping muscle is the one change that helps weight, bones and blood sugar at once.

### Questions readers ask

Does menopause cause weight gain?

Not directly. Ageing and lifestyle account for most of it, while menopause moves fat toward the waist.

Does HRT make you put on weight?

A Cochrane review of 28 trials found no difference in weight between women on HRT and women not taking it.

Should I take a supplement after menopause?

UK health advice is that all adults consider taking vitamin D every day, especially in autumn and winter. Ask your GP about calcium if your diet is low in it.
